Immunological Fecal Occult BLood

Hey guys!

Actually I promised you guys to continue the part II of my urine FEME, but I realised Azeimah had already done a very good job! Haha yup so think today I will share with you guys what I'm doing for my MP instead :)

I believe many of you guys had read on quite a few posts on testing for fecal occult blood(OB) using the guaic-based stool card. This is a screening test to detect for bleeding from anywhere of the GIT, particularly for colorectal cancer.

For my MP, i'm researching on the incidence OB. But instead of using the chemical method of stool card, my lab uses a machine called OC-Sensor DIANA.

This is an immunological method that uses the principle of latex agglutination reaction and an optical measurement method. The method is quite simple actually... the stool usually arrives in an OB casette. After that we just need to place the casette into rack and run the machine to get the results!

How it works?
If a person suffers from OB, there will be present HbAo hemoglobin in the stool.
OC-Sensor DIANA uses a latex reagent, which is prepared by sensitizing anti-human HbAo antibodies to polystyrene latex particles, to detect for the hemoglobin in stool. Upon addition of stool sample to the latex reagent, a latex agglutination will result due to the reaction of sensitized HbAo antibodies to HbAo hemoglobin in the stool sample.

Sensitized HbAo antibodies(in latex reagent) + HbAo hemoglobin(in stool) = latex agglutination

This reaction is then analyzed accordingly to the change in optical density. The higher the concentration of HbAo in the sample, the greater the change in the optical density.

Why is immunological method better?
-Unlike the stool card that indicates positivity through colour changes, OC-Sensor DIANA is able to provide an exact value of the amount of OB in the stool. If the optical density > 100 ng/mL, it indicates that the person is suffering from OB. Hence with the exact value known, we can tell whether the patient is suffering from mild or serious fecal occult bleeding.

-If using the stool card, a patient need to avoid a variety of food 24-72hrs before the test to prevent false results. This is because the hydrogen peroxide used to detect for OB can be interfered by many food. For OC-Sensor DIANA, no dietary restriction is required.

-For OC-Sensor DIANA, the sample can be kept for 21 days if refrigerated. But for stool card, if the stool is harderned, the colour change will not be obvious.

Disadvantages of immunological method....
The only disadvantage of using OC-Sensor DIANA is that the test must be performed in laboratory. For stool card, if you have the necessary materials you can simply perform the test yourself at home. So the stool card is a more conveneint method.

This immunological method (OC-Sensor DIANA), issit based on the same principle as OC light (mentioned in alex and sofie posts)????? Cause from what I read, OC light is also based on haemoglobin and antibodies. Or are they the same thing?

hi,

i've always wanted to try testing stools but sadly, the lab i'm attached to does not handle stools specimens. so i cant really imagine how OB is done.

anyway, u mention that oc-sensor diana is a machine right? hmmm, is it a big analyser or is it just like a test kit? can you share with us how the test is performed? (the manual steps?) and can you upload some picture for me to visualise? =X

Hey Xin Ni!

Hmmm its not the same as alex and sofie's method... their result is still based on the colour change of test strip but mine will give an exact value...

The other difference is that from sofie's blog it says her OC light method can detect 10µg to 100mg/g of feces.. mine can detect 10ng to unlimited amount :)

Haha not a prob... sorry for not elaborating clearly:P

Its a analyser.. not very big actually:) Its very easy.. the stools now come in the form of casettes instead of stool card. So we just need to put the casettes onto the rack and run the machine!

Its really a convenient method. Usually my collegues will place all the casettes into a container for me when distributing the specimens.. so when i see theres lots of OB casettes i will just load them onto the rack and run the machine. While waiting for the results i will do other stuff... so the whole process usually will take me only around 5 mins(which is the loading time) :)

since OC-Sensor DIANA uses the optical measurement method to obtain the values, will lipaemic samples result in false positives? if not, wat kind of samples will result in false results?

Hey Malerie!

OC-Sensor DIANA can detect stool up to 1000ng/mL... When its over the amount, we just need to put e OB casette onto the dilution rack instead of the normal rack and run the test again :) I have seen result up to ten-thousands ng/mL before!

There are several factors that cause false-positive results which affects both chemical and immunological methods:

• Use of anticoagulants, aspirin, colchicine, iron supplements, NSAIDs (anti-inflammatory analgesics), and corticosteroids in large doses causes GIT bleeding
• Bleeding of gum due to recent surgery of mouth
• Nosebleeds
• Menstruation

HELLO HUIMIN!

I've been wanting to ask you so what is inside the cassette that allows the stool to be still in good condition even after collection (like it will not harden)

How much dilution will you carry out for the positive cases?

Hey lesile!

Erm actually im not sure what e liquid inside the cassette is called but its allows saturation of the stool to ensure a homogenous solution:)

Because the stool is now saturated and not in a clump... tts why it wont hardern easily and thus we can preserve longer:)

The dilution defers according to each sample... the highest dilution factor that the machine can perfom is 250 times :)

Hi SIP,
I am preparing to use OC-Sensor Micro for my lab. so I am wondering about reagents of this machine such as: OC Auto III Latex, Buffer, HbAo standard, Control L&H. one time I order is 3000 tests so how many boxes of every kind are enough for 3000 tests, and how to save these reagents?
I am very glad to find a person who has much knowledge of this instrument like you and I hope you will help me this answer!
